Each adult participant must complete the adult form. A parent or legal guardian must complete the minor form for each participant under 18. Coaches, organizers and facility renters must also complete the coach/renter form, which does not replace individual participant forms.

Alcohol, drugs and tobacco products are strictly prohibited anywhere on Northgate property, indoors or outdoors. The consumption, possession or distribution of these items is not permitted. This policy helps maintain a safe, healthy and respectful environment for athletes, families and visitors. Violations are subject to disciplinary action.
Video surveillance operates 24/7.
Northgate property is monitored and recorded around the clock. Video surveillance helps deter prohibited activity, document incidents and protect visitors, staff and property. Its use reflects Northgate’s commitment to maintaining a secure environment for everyone on the premises.
